How soon can I get an appointment for urgent care needs?
We offer same-day appointments for urgent medical concerns. Simply call our office or use our online booking system and indicate that your visit is urgent. We reserve several appointment slots each day specifically for patients who need immediate attention. For life-threatening emergencies, please call 911 or visit the nearest emergency room.
What should I bring to my first appointment?
Please bring a valid photo ID, your insurance card, a list of current medications (including dosages), any relevant medical records from previous providers, and a list of questions or concerns you'd like to discuss. If you're taking medications, bring the actual bottles or photos of the labels. Arrive 15 minutes early to complete any necessary paperwork.
Can I switch from in-person to online consultations?
Absolutely! Many of our services can be provided through secure video consultations. After your initial in-person visit, follow-up appointments, prescription refills, mental health counseling, and many other services can be conducted online. Simply let us know your preference when scheduling, and we'll send you a secure link to join your virtual appointment.
What are your office hours and location?
We're located at 2856 Lotus Pl, Loveland, Colorado 80537. Our regular office hours are Monday-Friday 8:00 AM - 6:00 PM, Saturday 9:00 AM - 2:00 PM. We offer extended hours for working patients and 24/7 phone support for urgent concerns. Online consultations are available seven days a week, including evenings and weekends.
Do you provide services for children and seniors?
Yes! We provide comprehensive healthcare for patients of all ages, from newborns to seniors. Our family medicine physicians are trained in pediatric care, adult medicine, and geriatric health. We offer well-child visits, immunizations, adolescent health services, adult physicals, and specialized senior care including chronic disease management and wellness programs.
How do I access my medical records and test results?
We provide secure online patient portal access where you can view your medical records, lab results, immunization history, and appointment schedules. You'll receive login credentials after your first visit. The portal also allows you to message your healthcare provider, request prescription refills, and pay bills online. All information is encrypted and HIPAA-compliant.
What is your cancellation and rescheduling policy?
We understand that life happens. If you need to cancel or reschedule, please notify us at least 24 hours before your appointment. This allows us to offer the time slot to another patient. Late cancellations or no-shows may incur a fee, especially for specialized appointments. You can cancel or reschedule by phone, through our patient portal, or by replying to your appointment confirmation email.
